Forms & Permit Applications

The two most common permit applications filed with the Conservation Commission are Requests for Determination of Applicability (RDA) and Notices of Intent (NOI). Instructions on how to complete these applications, including what supporting documentation is required, can be found on this page. Incomplete applications will not be processed. Please refer to the checklist enclosed in both the RDA and NOI packets to ensure that submissions are completed before submitting. See the Conservation fees for requirements.

When filing these documents in accordance with the regulatory requirements, please send the emailed copy at the same time. conservation@spencerma.gov

NOTE: Any plan revisions must be submitted at least seven (7) days prior to the relative public hearing date in order to allow for review by the Commission and any interested party. Plans submitted less than seven (7) days from the scheduled public hearing may result as unacceptable by the Commission and/or delay in permitting the project.
